Quality of red blood cells washed using the ACP 215 cell processor: assessment of optimal pre- and postwash storage times and conditions.

Purpose of Paper

The purpose of this paper was to determine the effects of storage solution and pre- and postwash storage on levels of supernatant potassium, adenosine triphosphate (ATP) and hemolysis in red blood cells (RBCs).

Conclusion of Paper

Washed RBCs in saline-adenine-glucose-mannitol (SAGM) or additive solution 3 (AS-3) had significantly lower hemoglobin (Hb) and supernatant potassium levels than unwashed RBCs. Hemolysis and supernatant potassium increased while ATP levels decreased with pre- and postwash refrigerated storage, but the increase in hemolysis was greater in washed RBCs than unwashed RBCs. The hematocrit and mean corpuscular volume (MCV) were higher, and hemolysis was lower in washed RBCs stored in AS-3 compared to those stored in SAGM. RBCs stored in SAGM showed slower declines in ATP levels during postwash refrigerated storage than those washed and stored in AS-3. Based on the data, a prewash refrigerated storage time of no more than 14 days was recommended.

  1. Study Purpose

    The purpose of this study was to determine the effects of pre- and postwash refrigerated storage and storage in AS-3 rather than SAGM on levels of supernatant potassium, ATP and hemolysis in RBCs. Blood was collected into citrate-phosphate-dextrose (CPD) tubes and held overnight before processing by the buffy coat method. RBCs were then obtained by the buffy-coat method, suspended in SAGM and washed using an ACP215 cell processor on day 2, 7, 14 or 21 and resuspended in SAGM or AS-3.

    Summary of Findings:

    Washed RBCs in SAGM or AS-3 had significantly lower Hb content than unwashed RBCs (p<0.01 and p=0.03, respectively), but there was no difference in Hb content between washed RBCs stored in SAGM and AS-3. The hematocrit and MCV were significantly higher in post-wash RBCs suspended in AS-3 than SAGM (p<0.01, both). Hemolysis was higher in RBCs washed and stored in SAGM than those stored in AS-3 (p=0.01). Hemolysis increased with pre- and postwash refrigerated storage, but the increase was greater in washed RBCs than unwashed RBCs (p<0.01). Further, the time at which the increase was significant depended on the total and individual durations of prewash and postwash refrigerated storage, with significant effects found after 28 days of combined refrigerated storage (7 days prewash and 21 days postwash or vice versa) in AS-3, after 7 days prewash and 7 days postwash in SAGM, or after 2 days prewash and 28 days postwash in SAGM. Supernatant potassium increased with refrigerated storage, but was removed by washing in SAGM or AS-3. 7 days after washing, supernatant potassium levels were higher than those found prewash on day 2. ATP levels declined with refrigerated storage of RBCs in SAGM or AS-3, but RBCs in SAGM showed slower declines in ATP levels during postwash refrigerated storage than those stored in AS-3 (p<0.01). Based on the data, a prewash refrigerated storage time of no more than 14 days was recommended.
