Purpose of Paper

This paper compared the diagnostic performance of 21-gauge (G) and 22-G endobronchial ultrasound-guided transbronchial needle aspirations (EBUS-TBNA).

Conclusion of Paper

EBUS-TBNA specimens obtained with 21-G and 22-G needles had comparable negative predictive value, and accuracy for malignant disease.  However, 21-G needles were better than 22-G needles for characterization of benign and non-small cell lung cancer (NSCLC) specimens.

  1. Study Purpose

    This study compared the diagnostic performance of 21- G and 22-G EBUS-TBNA. EBUS-TBNA specimens were obtained from 303 patients (175 with 21-G needle and 128 with 22-G needle) with unexplained mediastinal lymphadenopathy. For each location, 2 or more passes were obtained, fixed in formalin, and H&E and immunohistochemically stained. Staining was evaluated by lung histopathologists who were blinded to needle gauge. Final diagnosis of NSCLC, small cell lung cancer, metastatic cancer, lymphoma, sarcoidosis, tuberculosis, anthracosis, cyst, reactive nodes, or foamy macrophages was based on 12 months of follow-up.

    Summary of Findings:

    EBUS-TBNA specimens obtained with 21-G and 22-G needles had comparable sensitivity (93.9% and 92.1%, respectively), negative predictive value (92.7% and 89.7%, respectively), and accuracy (96.6% and 95.3%, respectively) for malignant disease.  However, 21-G needles were better than 22-G needles for characterization of benign (83% versus 60%, P<0.01) and NCSLC specimens (88% versus 65%, P<0.01).
