Purpose of Paper

The purpose of this paper was to investigate the reliability of the iSTAT portable clinical analyzer for analysis of pediatric blood specimens.

  1. Study Purpose

    The purpose of this study was to determine the effects of analyzer type on clinical chemistry and hematology analysis of pediatric blood specimens.

    Summary of Findings:

    The day to day coefficient of variance (CV) using iSTAT for electrolytes, glucose, urea nitrogen and pH was less than 2% and the CV for oxygen and carbon dioxide were less than 6.5%, but the hematocrit and creatinine were highly variable with CVs of 21 and 23.6%, respectively. The CVs using iSTAT were comparable to the CVs using the VITROS or ABL analyzers for all analytes except creatinine and hematocrit which had CVs of <3% using the VITROS analyzer and <9.1% using the ABL analyzer, respectively. The different cartridges performed similarly for all of the analytes that were analyzed by multiple cartridge types. However, a follow-up study of creatinine levels found less correlation between VITROS values using a new lot of cartridges, resulting in possible clinical differences. In specimens analyzed using iSTAT, a small proportional negative bias was found for ionized calcium (iCa) and partial pressure oxygen (PO2), and a there was a positive bias for creatinine. The iSTAT analyzer was found to have good reproducibility for children under 3 months old and those over 3 months old. The authors conclude that iSTAT is suitable for clinical chemistry and hematology analysis for all analytes except creatinine.
